PARENTAL/GUARDIAN CONSENT, DIGITAL WAIVER, RELEASE OF LIABILITY, AND OPTIONAL MEDIA AUTHORIZATION
Please read this document carefully. It contains important information regarding simulated-trading education, youth mentoring, online participation, digital privacy, photography and video use, assumption of risk, and limitations of liability.
This agreement is governed by the laws of the State of Georgia, except where applicable federal law or another mandatory law requires otherwise.

Student Full Name: __________________________________________
Student Grade Level (Grades 9–12): ___________________________
Parent/Legal Guardian Full Name: ______________________________
Parent/Guardian Email Address: ________________________________
State/Country of Residence: __________________________________
2. Parent or Legal Guardian Authority and Consent
I, the undersigned, certify that I am the parent or legal guardian of the minor student identified above and have the legal authority to provide consent on the student’s behalf.
I grant permission for my child to enroll in and participate in the four-week youth mentorship program, courses, simulated-trading activities, community discussions, live Zoom webinars, in-person events, and other approved educational activities provided by MEnYOUTH Wealth Academy, LLC (“MEnYOUTH,” the “Company,” “we,” “us,” or “our”).
I understand that the program includes two general areas of instruction:
Financial Literacy and Simulated-Trading Education
Instruction may include:
General financial-literacy concepts;
Futures-market terminology;
Chart reading and market structure;
Simulated order entry;
Risk-management concepts;
Trading discipline and patience;
Trade planning and journaling; and
Review of hypothetical or simulated trading examples.
Youth Mentoring and Development
Instruction and group discussions may include:
Character development;
Leadership;
Personal responsibility;
Goal-setting;
Peer pressure;
Cyberbullying;
Digital safety;
Online conduct; and
Digital-footprint management.
I understand that youth mentoring is educational and developmental. It does not constitute psychological counseling, psychotherapy, medical treatment, clinical assessment, legal advice, crisis-intervention services, or another form of licensed professional care.
I understand that I should seek assistance from a qualified professional when my child requires mental-health, medical, legal, academic, or emergency services.
3. Simulated Trading Only
I understand and acknowledge that all trading-related activities provided by MEnYOUTH Wealth Academy, LLC are conducted exclusively through simulated trading environments using simulated funds, commonly referred to as “paper trading.”
No real money, real capital, live brokerage account, or actual trade execution is used or permitted as part of the program.
Although instruction may use current, delayed, or historical market charts and pricing data, every order, trade, account balance, profit, loss, and performance result generated during the program is simulated.
MEnYOUTH Wealth Academy, LLC does not:
Conduct, host, supervise, facilitate, or participate in live, real-money trading;
Accept or manage participant funds for trading;
Pool participant funds;
Open, fund, connect to, access, control, or monitor brokerage accounts;
Place, execute, modify, or close live trades for participants;
Provide live funded trading accounts;
Exercise trading authority on behalf of a participant;
Share in a participant’s trading profits or losses;
Provide trade signals or trade alerts;
Provide copy-trading services;
Tell participants when to enter, exit, modify, or hold a live trade; or
Provide personalized live-trading recommendations.
Any chart analysis, trade setup, entry point, stop level, profit target, order type, or risk-management example shown during instruction is presented solely as an educational example within a simulated environment.
Such examples must not be interpreted as a signal, alert, recommendation, promise, or instruction to place a live trade.
MEnYOUTH Wealth Academy, LLC does not facilitate, supervise, encourage, or authorize real-money trading by minor participants.
The Company will not:
Open or arrange a live brokerage account for a minor;
Accept money from a minor for trading;
Place live trades for or with a minor;
Connect a minor’s simulated account to a live brokerage account;
Provide individualized live-trading instructions to a minor; or
Encourage a minor to bypass brokerage eligibility requirements, parental controls, platform rules, or applicable laws.
Any financial account created outside the Company’s program is solely the responsibility of the parent or legal guardian and the applicable financial institution.
Such an account is not sponsored, supervised, recommended, operated, funded, or managed by MEnYOUTH Wealth Academy, LLC.
4. No Financial Advice or Advisory Relationship
I understand that MEnYOUTH Wealth Academy, LLC provides general education only.
The Company does not provide personalized investment, financial, brokerage, accounting, tax, or legal advice.
Nothing provided through the website, classes, mentorship sessions, presentations, recordings, simulations, social-media pages, community discussions, or educational materials constitutes:
A recommendation to buy, sell, hold, or trade a futures contract or other financial instrument;
Personalized advice based on a participant’s financial circumstances, goals, experience, or risk tolerance;
A promise or guarantee of profits, income, financial independence, employment, or investment success;
An offer to manage money or operate a trading account; or
An offer to act as a broker, investment adviser, commodity trading adviser, account manager, financial planner, or custodian.
Participation in the program does not create a fiduciary, investment-advisory, commodity-trading-advisory, brokerage, attorney-client, tax-advisory, or other professional advisory relationship between the participant and the Company.
5. Simulated and Hypothetical Performance Disclosure
I understand that any charts, simulated account balances, screenshots, demonstrations, recordings, testimonials, trade examples, or performance figures displayed by the Company may reflect hypothetical or simulated results.
Such information is provided for educational and illustrative purposes only.
Simulated trading does not involve the same financial pressure, emotional responses, execution conditions, liquidity, slippage, fees, commissions, margin requirements, technology interruptions, or other factors that may affect actual trading.
The following disclosure applies whenever simulated or hypothetical trading results are displayed:
HYPOTHETICAL OR SIMULATED PERFORMANCE RESULTS HAVE CERTAIN INHERENT LIMITATIONS. UNLIKE AN ACTUAL PERFORMANCE RECORD, SIMULATED RESULTS DO NOT REPRESENT ACTUAL TRADING. ALSO, SINCE THE TRADES HAVE NOT ACTUALLY BEEN EXECUTED, THE RESULTS MAY HAVE UNDER- OR OVER-COMPENSATED FOR THE IMPACT, IF ANY, OF CERTAIN MARKET FACTORS, SUCH AS LACK OF LIQUIDITY. SIMULATED TRADING PROGRAMS IN GENERAL ARE ALSO SUBJECT TO THE FACT THAT THEY ARE DESIGNED WITH THE BENEFIT OF HINDSIGHT. NO REPRESENTATION IS BEING MADE THAT ANY ACCOUNT WILL OR IS LIKELY TO ACHIEVE PROFITS OR LOSSES SIMILAR TO THOSE SHOWN.
Past, demonstrated, hypothetical, or simulated results do not guarantee future performance or success.
6. Futures-Trading and Leverage Risk Acknowledgment
I understand that although live trading is not included in the program, futures trading with real money involves substantial risk and is not appropriate for every person.
A person who independently chooses to participate in real-money futures trading outside the Company’s program may lose some or all deposited funds.
Depending on the product, brokerage agreement, market conditions, leverage, and margin requirements, losses in a live trading environment may exceed the amount initially deposited.
Success in a simulated environment does not guarantee, predict, or necessarily correlate with success in a live trading environment.
Any real-money trading or other financial activity undertaken outside the Company’s program is independent of MEnYOUTH Wealth Academy, LLC and is not instructed, supervised, monitored, approved, funded, or guaranteed by the Company.
7. Online Webinar Participation and Digital Privacy Safeguards
The Company may provide online instruction through third-party platforms such as Zoom Video Communications.
Student Cameras and Microphones
To protect the privacy of minor participants, student cameras and microphones will remain disabled during Zoom webinars.
Students must not independently activate their cameras, microphones, screen-sharing functions, or other audiovisual features without express authorization from the Company.
When participant interaction is permitted, students may be allowed to submit questions through moderated chat features, polls, or other Company-approved tools.
Webinar Recordings
Live Zoom webinars may be recorded for educational playback by properly enrolled participants.
Webinar recordings are intended to capture only:
The instructor or authorized Company representative;
The instructor’s voice;
Educational presentations;
Charts;
Simulated-trading demonstrations; and
Other Company-approved instructional materials.
The Company does not intend to capture or publicly display a student’s photograph, video, voice, home environment, or other personal identifying information during Zoom webinars.
The Company will use reasonable platform settings and administrative procedures to prevent participant cameras, microphones, private messages, email addresses, and other identifying information from appearing in educational recordings.
Student display names, submitted questions, poll responses, or chat messages may be visible to Company staff during a live webinar.
The Company will use reasonable efforts to remove, conceal, or avoid displaying personally identifying student information before making a recording available for later educational playback.
Recordings containing identifiable information concerning a minor will not be used for public advertising or promotional purposes without separate written or electronic permission from the student’s parent or legal guardian.
Moderated Interaction and Online Conduct
Interactive chat features, community discussions, and other online communications may be monitored and moderated by Company representatives.
These measures are intended to help identify and address:
Cyberbullying;
Harassment;
Threatening or inappropriate behavior;
Disclosure of personal information;
Disruptive conduct;
Inappropriate language or content; and
Unauthorized communications between participants.
Although the Company will use reasonable moderation and digital-safety procedures, it cannot guarantee that every inappropriate communication, technical issue, or unauthorized action will be prevented.
Participants may not record, photograph, screenshot, livestream, copy, or distribute:
A webinar;
Another participant’s information;
Private chat messages;
Company presentations;
Simulated-trading demonstrations; or
Other Company instructional materials
without prior written permission from the Company.
Digital Information
I consent to the Company’s collection, use, processing, and storage of the registration and participation information reasonably necessary to:
Administer the program;
Verify participant eligibility;
Communicate with students and parents;
Provide educational services;
Maintain program security;
Process enrollment and payment;
Monitor participation and attendance; and
Comply with applicable legal obligations.
Information will be handled in accordance with the Company’s Privacy Policy and applicable privacy laws.
8. Participant Conduct
I understand that my child must participate lawfully, respectfully, and appropriately.
My child may not:
Harass, threaten, intimidate, or bully another person;
Share discriminatory, unlawful, deceptive, or inappropriate content;
Disclose another participant’s private information;
Share class links, passwords, recordings, or materials with unauthorized persons;
Attempt to access a class or resource for which the student is not registered;
Misrepresent simulated results as verified live-trading results;
Represent that personal trading activity is managed, supervised, approved, funded, or guaranteed by the Company;
Use Company materials to provide trade signals, financial-advisory services, or copy-trading services;
Copy, sell, publish, teach from, or redistribute Company materials without written permission; or
Interfere with the safety, privacy, integrity, or learning environment of the program.
The Company may suspend or remove a participant whose conduct threatens safety, privacy, program integrity, or the learning environment.
9. Safety Concerns and Communications With Parents
I understand that the Company may communicate reasonable safety, behavioral, or participation concerns regarding my child to me or another authorized parent or legal guardian.
The Company may report information to appropriate authorities when it reasonably believes a report is required by law or necessary to address an immediate and serious safety concern.
The Company does not promise confidentiality for information indicating possible:
Abuse;
Neglect;
Exploitation;
Unlawful conduct; or
A serious threat to the safety of a participant or another person.
10. Optional In-Person Photography, Video, and Media Authorization
I understand that photography, video recording, or audio recording may occur during certain in-person classes, workshops, presentations, ceremonies, community events, or other Company activities.
Media may be captured for lawful educational, informational, marketing, and promotional purposes, including possible use in:
The Company’s website;
Company-managed social-media accounts;
Printed brochures and flyers;
Email communications;
Digital advertisements;
Presentations;
Sponsorship or partnership materials;
Newsletters;
Program reports; and
Other Company promotional materials.
Media Consent Is Optional
Permission to use my child’s image, likeness, voice, or recorded statements for promotional purposes is optional.
Declining promotional media permission will not, by itself, prevent my child from enrolling in or participating in the program.
Please select one of the following options:
☐ YES — I AUTHORIZE PROMOTIONAL MEDIA USE
I authorize MEnYOUTH Wealth Academy, LLC and its authorized representatives to photograph, video record, or audio record my child during approved in-person Company activities.
I authorize the Company to edit, crop, reproduce, publish, display, and distribute approved photographs, videos, or audio recordings featuring my child for the educational, informational, marketing, and promotional purposes described above.
I understand and agree that:
The Company may use approved media without additional approval or compensation;
The Company may edit or format the media, provided it is not used to materially misrepresent my child;
The Company will not publish my child’s full name with promotional media unless I provide separate written authorization;
The Company will not sell or license my child’s identifiable media to an unrelated third party for that third party’s independent commercial use;
This authorization remains effective until I withdraw it in writing; and
Withdrawal will apply only to future uses after the Company has received and reasonably processed my request.
☐ NO — I DO NOT AUTHORIZE PROMOTIONAL MEDIA USE
I do not authorize the Company to intentionally photograph, video record, audio record, or publish identifiable promotional media featuring my child.
I understand that the Company will make reasonable efforts to keep my child out of intentionally captured promotional media.
My child and I agree to follow reasonable instructions concerning:
Designated seating;
Photography or recording areas;
Identification procedures;
Check-in procedures; or
Other arrangements intended to honor this selection.
If my child is incidentally captured in the background of a photograph or video, the Company will not intentionally publish an identifiable image of my child for promotional purposes without obtaining permission or taking reasonable steps to crop, obscure, or remove my child from the media.
Withdrawal of Media Permission
A parent or legal guardian who previously granted media permission may withdraw permission for future promotional use by submitting a written request to:
menyouthwealthacademy@gmail.com
A withdrawal will apply prospectively after the Company has received and reasonably processed the request.
I understand that withdrawal may not require the Company to:
Recall or destroy materials previously printed or distributed;
Remove content already lawfully published or archived;
Retrieve copies independently downloaded, saved, or reshared by another person;
Remove media distributed to a third-party platform before the withdrawal was received; or
Remove media in which my child is no longer identifiable.
11. Participant and Guest Recording Restrictions
Participants, parents, guardians, and guests may not photograph, video record, audio record, livestream, screenshot, or publish:
Another participant;
A private class or mentorship session;

Technology interruptions;
Internet or software malfunctions;
Unauthorized conduct by another participant;

Misunderstanding or misuse of educational information;
Travel to or attendance at an in-person event; and
Other risks ordinarily associated with educational and group activities.
I voluntarily assume responsibility for my child’s participation, subject to rights and protections that cannot legally be waived.
I acknowledge that performance in a simulated trading environment does not guarantee success in a live market.
13. Release and Limitation of Liability
To the fullest extent permitted by applicable law, I release, waive, and hold harmless MEnYOUTH Wealth Academy, LLC and its owners, officers, directors, employees, contractors, instructors, mentors, representatives, and agents from claims, liabilities, losses, or damages arising from or related to:

This release does not apply to liability that cannot legally be waived or limited.
Nothing in this agreement releases the Company from liability to the extent such a release is prohibited by applicable law.
14. No Guarantee of Outcomes
I understand that MEnYOUTH Wealth Academy, LLC does not guarantee:
Trading profits;

Any particular financial, educational, personal, or professional result.
Results depend on factors outside the Company’s control, including participation, attendance, effort, judgment, personal circumstances, technology, and individual decision-making.

Video-conferencing platforms;
Learning-management systems;
Email providers;
Payment processors;
Charting software;
Market-data providers; and
Simulated-trading platforms.
The Company does not control and cannot guarantee the uninterrupted availability, security, accessibility, or performance of third-party services.

Third-party services may be governed by their own terms, privacy policies, eligibility requirements, and platform rules.
