PARENTAL/GUARDIAN CONSENT, DIGITAL WAIVER, AND RELEASE FORM

Please read this document carefully. It contains important legal disclosures regarding simulated trading education, digital media usage, and limitations of liability. Because MEnYOUTH Wealth Academy, LLC is based in Georgia, this agreement is governed by the laws of the State of Georgia, and applies globally to all participants regardless of their physical location.

2. Scope of Services and Consent

I, the undersigned, am the parent or legal guardian of the minor student named above. I hereby grant permission for my child to enroll and participate in the 6-week mentorship program, courses, community groups, and live Zoom webinars hosted by MEnYOUTH Wealth Academy, LLC (the "Company").

I understand that the curriculum covers two distinct areas:

  • Financial Literacy: Instruction on how the global financial markets work, specifically focused on trading futures contracts.

  • Youth Mentoring: Interactive instruction and group discussions regarding modern digital safety, handling peer pressure, preventing cyberbullying, and managing a positive digital footprint.

3. Mandatory Simulated Trading Disclosure (CFTC Rule 4.41)

I explicitly acknowledge and understand the following conditions regarding the financial literacy portion of the academy:

  • Strictly Simulated: All trading activities, software, platforms, and exercises provided by the Company are 100% simulated ("paper trading"). No real money, real capital, or live brokerage accounts are used or permitted.

  • No Financial Advice: The Company is an educational entity, not a licensed brokerage or financial advisory firm. No content shared constitutes a recommendation to buy or sell real assets.

  • Regulatory Warning: Pursuant to Commodity Futures Trading Commission (CFTC) Rule 4.41, I acknowledge that simulated or hypothetical performance results have inherent limitations, do not represent actual live trading, and are not a guarantee or predictor of future real-world financial success. Futures trading involves substantial risk of loss and is not suitable for everyone.

4. Online Webinar Participation & Digital Privacy Safeguards

Because the Company delivers services globally via the internet and third-party software (such as Zoom Video Communications), I consent to the following digital environment protocols:

  • Privacy Settings: To protect minors, live webinars are configured so that attendee cameras and microphones are disabled by default.

  • Moderated Interaction: Any interactive chat features or community forums are heavily moderated by Company mentors to eliminate cyberbullying, peer pressure, or inappropriate behavior.

  • Recording Acknowledgment: I understand that live webinars may be recorded for subsequent educational playback by enrolled students. No personal identifying information of my child will be published or broadcasted publicly without separate written consent.

  • Cross-Border Data Consent: I consent to the secure collection, processing, and storage of my child's basic digital registration data (name, email, age) in accordance with the Company’s Privacy Policy, which aligns with federal COPPA guidelines and international data minimization standards.

5. Legal Waiver, Release of Liability, and Governing Law

  • Assumption of Risk: I assume full responsibility for my child's participation in this online program. I recognize that success or high performance in a simulated "paper trading" environment does not translate to success in live markets.

  • Release of Liability: To the maximum extent permitted by applicable law, I release, waive, and hold harmless MEnYOUTH Wealth Academy, LLC, its officers, directors, employees, mentors, and agents from any and all claims, liabilities, losses, or damages (including psychological, financial, or technical) arising from my child's use of the website, participation in Zoom webinars, or reliance on educational materials.

  • Georgia Jurisdiction: This agreement is executed, delivered, and intended to be performed in the State of Georgia. I agree that any legal dispute, claim, or interpretation of this waiver shall be governed strictly by the laws of the State of Georgia, without regard to its conflict of law principles, and shall be resolved in a court of competent jurisdiction located in Georgia.
